Forbidden Siren 1/2 (via emulator) - the most scary for me from them all, and also it's stealth based and requires planning and experimentation, you'll be scared playing it even if you're Chuck Norris, guaranteed.

Penumbra 1/2
Dead Space (?)
Scratches (probably the only point and click adventure that I found quite scary)

Undying's demon spawnfest spam mechanic just got tedious at some point. Dead Space was never really scary, apart from the *boo* moments, it was mostly about tension and release a lot of the time.
Call of Cthulhu: Dark Corners of the Earth is a worthy recommendation. SS2 and T: DS's Cradle level are also great at creeping you out completely.
I'd recommend SH1 for the unrelenting sense of creeping dread, and 2 for the story. You're never ever left feeling comfortable in the good SH games. It's the unease that makes them.

Seconded for the Penumbra games. They're not only scary, but near the end of Black Plague when you meet up with........... I CAN'T BELIEVE HE DID THAT!
Also, "shaft 13". When you see that sign on the wall in Overture, prepare to be scared. :)
I love games where the music changes when enemies attack... especially when I have no idea where the enemy is... I just know it is coming for me!
Reminds me of roaming around the ocean labs in DX and hearing the fast music start playing as a giant ********* comes to attack.

Vampire: Bloodlines. Only a few levels are scary per se, but the game is so great that it deserves recommendation everywhere anyway.

Oh, in that case I'm gonna go ahead and drop an obligatory mention of S.T.A.L.K.E.R. Can't comment on the other two yet, but Shadow of Chernobyl kept me fairly on edge for most of the time, and some of the underground sections were beyond intense. Granted, I'm by no means an expert in horror
games anythings, but X18 was the scariest level I'd played since Thief 3's Shalebridge Cradle. And even above ground some of those random night time encounters creeped the everloving shit out of me.

Realms of the Haunting isn't necessarily Scary i think.. although it is a really great game. i would recommend it if you can manage to get it somehow. Runs fine in DosBox with the cd roms.
and dont discount avp3 because of what you've heard. The marine campaign is by far the most developed of the 3, and it's great. The whole game might fall short of its predecessors in several aspects, but it is by no means unplayable.

While Clear Sky is noticeably lacking (except for one section), Call of Pripyat is
loaded with scary, far more than even Shadow of Chernobyl.
