Just gonna bump this, don't mind me...
Anyways, I'm currently at the end of the cave system that you enter from the thieves' hideout, it's a large room with a great metal gate and a lever near it that i can't frob. There appears to be nothing to do in this room, but it looks too important to just be decoration. Am I missing something here?

It's the cave room where you watch two people killing each other.
There appears to be nothing else there.

Based on my loot list from a few years back, there's one on a concrete wall in cave with lava pool, and one under piece of rubble in cave beyond the portal.

what are the enemies on the red section, where the lava is? Are they steam? because I shot a water arrow at the one in the entrance and the smoke/steam? went out. Are they killable? I've tried broad heads, water, sword... I die too soon with sword, bj,

Smoke shadows? Try moss arrows, there is a hint in a room with a skeleton in it.
It seems that you overcame your fear of zombies, didn't you? :D

Backstabbing them with sword can work, but you take damage if you get too close (even when they don't notice you).
