Also, when GRRM was asked during an interview about Jon Snow's death, he replied "Oh, you think he's dead, don't you ?". GRRM then refused to discuss whether Jon Snow is dead or not. That is a clear indication to me that he's not just dead yet.

as jon dies, his oath to the black watch dies with him. Which is why his death was kind of inevitable.I've never been a fan of GRRM. I found his writing very base, maybe even smutty at times when theres just no need for it, doesn't add to the story in any way and...well, others have argued this before so I'll leave it at that.
One problem I have with him, related to the OP, is that he tries to too hard to be shocking.

I dunno, Martin has set out to very deliberately create a bleak, unfair and definitely not-sanitised kind of alternate-medieval-europe and anything shocking seems fitting given that context.
ok there was a description of Tyrion's dick that I could have done without.
[edit]I do wonder if the sense of "GRRM could kill a protagonist at any moment" is exaggerated. Several characters are on arcs where, if they died it wouldn't just be shocking, it would mean we've totally wasted our time reading about them. Arya for example might not live to see the very end but her whole "train to be an assassin" thing is clearly going *somewhere*. Also if we lose Jon that's the major POV character at the wall gone, which would make continuing the plot there difficult.

Tried reading some of his books overseas, but couldn't get into them for exactly those reasons. Seemed to me like he was trying too hard in every aspect of the book.
To the original topic, I think one of the most effective ones is when protagonists....well, they survive because they're needed for the story but it can make things a lot more interesting when them damaged either physically or mentally. One small example I can think of is when Paul survives a major event in the second book, but is blinded as a result.
