There's definitely a bug with the lever sequence not always working.
The green sequence wouldn't lock in me for me.
I eventually managed to get past it by turning all the levers to the opposites of their correct positions, then turning them all back to the correct positions - which still didn't solve it - and then randomly flipped the middle lever back and forth in frustration and suddenly it locked in.
