But I think it's some kind of a smoke arrow : you throw it and it creates a big smoke cloud to cover your escape.

I'm trying to take into account the details given, and comparing it with the in-game information. The stem/handle is smooth, with that one joint, where it either spins or merely screws into the head. If it was meant to be an explosive, it could also be stuck into the ground. As an explosive, it just seems redundant with mines and fire arrows, unless it really was replaced by cannonballs.
I'll be happy to see a strong theory that disallows continuity...or hear an explanation from the artist. :p Even if the idea of a tiny, useful boiler has a place in the Thief Universe, that alone opens up a lot of possibilities. Neither this gizmo, nor the ScurryBots were implemented in-game, so the golden Cherubs have the smallest boilers in the canon.

Looks like a button on the bulbus end; a possible detonation switch?
The rod end does suggest an arrow, or hand held device. But without some form of fletching or hand grip, it is hard to tell.
I have slight doubt that it is fire-based. The glow may or may not be fire, as that may not be smoke, but steam coming from the bulbus end.
But in my humble opinion, I believe it to be a fire arrow concept.

I think it looks like a steam whistle you can find in Thief: The Metal Age...
It would suit on a boiler or whatever sort of steam driven machine.
