I see your point, but there are ways to emulate paddle for this purpose with sufficient fidelity. Splinter Cell series uses mouse wheel as a kind of "throttle". Works fine in my opinion.
However, I'm not convinced this kind of resolution is really useful in a stealth game. It didn't add much to Pandora Tomorow, anyway. Much depends on actual game design, but I don't think having more than three speeds in TDP or TMA would make a significant difference.
In general, gamepad is superior in games where your controls are "fuzzy", that is: you need a good freedom of movement, but you don't need much precision. Driving games are a good example. One-on-one fighting games, too, although I think this kind of gameplay is doable with m&kb. Unfortunately, FPP games are all about precision, because you often use camera movement to point at things (whether to use them or shoot them).

This is only due to bad design of player movement. To me, the confusion from looking at everything from "outside" was much worse than occasional fall from a narrow beam. This really does amount to "depends on your perception".
