The game has glitched on me too.
Finally reached level 4, where I died. It then let me continue from level 4 the next time I started (even though I hadn't saved), so I proceeded in what was a new randomly generated level 4 and found the teleporter... which took me to Level 3. couldn't then get back to Level 4, so I quit. It's now wiped the Monster database. Ho hum.

I'm in a tight spot with a sizeable swarm chasing me in a room, so I drop a nailbomb behind me as I run, and mutter a satisfied "yes!" under my breath as all but one of them perish in the blast of shrapnel. Sweet! The lone survivor is still after me; I fire my revolver at it, but it's still coming, so I fire again; still nothing. A different part of my brain interrupts to ponder the fact that in approximately the past second, a pair of familiar-looking red packages have travelled a short distance from my character in the general direction of the enemy.
Nailbombs. Ffffffffuuuuuu....
I uselessly start to run a split-second before they rip us both to shreds.
