Azaran - I believe that lever in the upper passage gives you a way to reach that button.
Sanctuary - look up at the two towers (in front of the gate).

Have you found your way down to the door leading to where the electrics and the spiders are? When you get there and look around and throw a switch, a passage will open and then you'll be able to move on.

I found the switch in the electrical room and opened up a passage to the underground room with all the fish (and saw Benny lose his head :sweat:), but the lever door is still locked :(

Do you mean the lever who is behind you when you stand in front of the opening by which you saw the death of the guard (Benny)?
This lever open a railling door. You can see it by watching by the opening.

Ok, I pulled the 3 levers in the ruins, sanctuary and temple, but I don't know where to go next. I assume there's a 4th lever I'm supposed to find (since that's the only objective left) but I can't find it. :(

It sounds like you're missing the one in the Children of the Black Frog area. Check your map. They are all marked on it. You'll need to pay a visit to the village in order to reach that area.
