This looks a bit rediculous, but with it you could play thief with only a mouse haha and also have analogue movement.
I think analogue movement is the main thing - something like that is what fps games miss out on on pc - not that the concept has ever been developed that highly.

It looks like it would be too easy to accidentally press the wrong button. They're all very close and it would be easy to just forget what each one does.
With a keyboard we can associate a proper key name (like A or Ctrl) with an action, and our familiarity with typing allows us to remember where those keys are. With that mouse the buttons will have names like 'button 4', which I think will be harder to remember.

Yes, it doesn't look the best design haha. I wonder if an analogue stick on a normal style mouse would work nicely though? Or would it be better added to a keyboard?
I guess it's more the concept than the implementation - but tiny, close together, unlabelled buttons aren't going to win any awards ;) Looks cheap and plasticy too.
