OK, I've got the spear and have used the key to enter The Mouth of Orodruin.
I saw the sign telling me to hop like a rabbit. not to mention the bouncy music.
All I see is these squares going down and eventually I'm at the lowest square and from there can only fall to my death. How am I supposed to traverse this area?

Stand on the top step and look down straight ahead.
The step you want is the lowest one you can see. So go down 7 steps (you need to jog sideways twice) then look behind you.

I have encountered a bug with the optional mission to rescue the mechanist from the jail cells and optionally trap her guards in the cell.
I don't know if she is supposed to be, but the prisoner is dead. Within 5 seconds of the mission activating it is GAME OVER and the two options have red X's through them.
Also, how do I take the elevator up past the 2nd floor to what is presumably the penthouse ?

malau, re: elevator: Use the silver gear on the gear lock, then the "up" button appears. As previously noted, the silver gear is something you make, not something you find.
Regarding the optional mission, I played thru that yesterday (didn't encounter the bug, though). It helps that my playstyle is to blackjack every human in sight. You get a dialogue where the guard and the prisoner trash-talk each other when you go around the corner in the prison, then once you get close to the prisoner's cell you get another dialogue with the prisoner where you get the objectives. I suspect the bug may be related to you doing something strange during one of those two dialogues. How I reached those objectives: I had already blackjacked one guard. Put that guard in Cell 2. Use the switches to open Cell 2. Shoot a noisemaker at Cell 2, then use the switches to close it once the non-blackjackable guard chases the noise arrow. With everyone else blackjacked, open Cell 1.

I'm really enjoying this mission. But I did finally get stuck. Im on the find the secret switch in the mentalists office. I have searched hig and low and can't find it. Any help appreciated!

I don't think the mentalist's office has the switch. It's the librarian's office that has the switch. That's the room near the bookcases with the safe.
