I recently read an article about an anti-virus software accidently flagging part of Windows as a potential threat, preventing it from loading and hence nuking lots of users' machines as a result. I'm not trying to single out the company that made the product, because if you look back through history this has happened to pretty much all of the AV companies at least once.
Even if you beta-test your updates/product carefully, sometimes shit will still happen. Its inevitable. The worst part about this was that it was the professionl version of the software that suffered from the glitch. The free version from the same company didn't have the bug... ouch! It really cracked me up.
