I've only played the Xbox version and I don't know how it scales, but there was definitely more enemies when there was more players. Didn't necessarily apply to events. The difficulty only scaled the amount of damage done and received.

Not that I know of. There is just a spawn flag in the editor for co-op so that you could place items/enemies that only appear there. An enemy multiplier was introduced in the original games through a mod called Parse Error, lots of fun to play with that. There is a similar mod for Serious Sam 2 called InSamnity.
The DLC is free for those who got both First and Second Encounter HD remakes. It immediately started downloading a 900 MB update when I logged into Steam. After that update you can just uninstall standalone First Encounter HD (Anything else would be a waste of space.). This DLC fuses the games together into Serious Sam HD, pretty much, which was Crotem's response to a lot of requests from the fans.

The models and textures shown in the teaser are unfinished, according to (one of) Croteam's artist, and I hope the same applies to the sound effects. I am very happy with the art direction though, that red light on the kamikazes' necks is going to haunt me in my sleep.
Also, in case you haven't heard, the game is scheduled for an August release.
