The clocktower guy - I don't believe is passable. There is also a guard in the family castle who walks up the stairs in Lady Elizabeth's tower, he faces away from you - but there is no way to get to the top without knocking him out.
I was just about to ask that. I can get past him without killing him, but it alerts half of that level, so knocking him out seems more logical.

Actually that one is doable. If you douse some torches you can flatten against the wall and if you keep moving he'll walk right by you as he is supposed to patrol back DOWN the stairs. But then when you go into Lady Elizabeth's room his patrol gets messed up and he stops at the top of the stairs. You can toss an object into the dark corner and sneak by when he goes to have a look. I suppose you can do that in a lot of cases, it's just annoying to have to.
I just remembered the most obvious and irritating place that's impossible to ghost - the Kurshok throne room. How on earth could you possibly get those chests open without a KO? Well, you can preserve your stats if you gas em, but not ghosting...

It's possible, but really really hard. You have to stand to the side of the chest so the guard can't see you, wait for him to turn away, pick the lock once, hide, repeat. You have to know which direction the sweet spots are though, so you can move the pick up there, click and have one done fast enough to hide again.

With regards to the city, I find Stonemarket Proper to be impossible to pass without racking up the bodies and catch stats. I normally go the long route - via the Docks-South Quarter gate.
Yeah, I suppose it's rare that you actually need to go to Stonemarket Proper after St. Edgar's. That's where alot of battles end up happening. But once the enforcers show up I think your stats are just plain screwed, because every townsperson who sees you assumes you're the culprit to any death, and any body discovered counts against you.

True, which is why I tend to 'break' ghost with enforcers just to get rid of them before they start damage. I also tend to hide bodies by loading/saving until I get it right. It also pays to learn all the 'catch' comments that are made, with subtitles - f.e. "you bes causing trouble". The catch script can be a little silly, particularly with factions that are suppose to be your friends.
Stonemarket Proper is the worst scenario - big pagan presence (where from?); medium hammer presence; female hooligans; city watch; (+enforcers +statues +gamall). Most of them don't like you, blame you if they spot you. I don't see why there are pagans in that area anyway, and it'd make more sense if it was dominated by Hammers, because of the clocktower and cathedral. I've played the area over a long period, there are definately more pagans spawning. I would expect to see Hammers patrolling Auldale, too.
You do need to go to Proper on the last day. It's a shame that Proper should be avoided as there is a lot of loot spawning there...
