I agree, I did put one like this in JailBreak but it was also hinted at on the map so...
I've found this in games outside the Thief realm also and it can be frustrating knowing you've found the secret but not actually finding it.

i don't remember in wich mission this happened to me but i'm pretty sure it wasn't jailbreak .
it was in a mission where you had to go and ransack Sheriff truart place . there where a lot of guards ' guarding that house. any way on the 2nd floor of that House , there was a switch next or close a fire place and i could actually hear the secret wall moving but i never found it .... i got in big trouble running around to try to spot where it might come from ... actually i got in trouble because i tried this about 12 times ... i hate running on noisy floors .... i finnally got rid of all obstructions hehehehehe
but i still never found where was that secret ...

Two more to add:
1. Spelling mistakes. I'm not too much a grammarian, but I notice spelling.
2. Maps that look like they were scrawled out in MS Paint. (Although it worked in Death of Garrett because it was funny and seemed like it should be that way.) I'd rather have no map at all than a badly drawn map. Even though I know I don't have to look at it I get a nagging feeling I'm toting something ugly around with me.
Anyway, no more grumpiness than that. I know making fan missions is a bear, I've been plodding at one myself.

Situations that don't make sense. For example, torches in crypts or sewers. Who lights these torches? If people go into these places to light them (who knows why?) then why don't they notice the undead and do something about it?
Another example: Monasteries or Cathedrals or Churches with lots of "guards" and not a lot of pure clergy.
One more: when the inside architecture and decor doesn't match the exterior.

Flat "cobblestone" ceilings are highly unrealistic (assuming the natural effects of gravity over time). Instead, they should be textured as wood, stucco, metal or some other convertional material. The only stone ceiling that makes any sense is an arched vault like in a church.
Lousy voice acting (I think this was mentioned already). Indeed, I know for certain that "volunteers" were dragged into the Looking Glass studio for some parts of Thief 2. FMs are generally quite good in this regard, although now that Loanstar has given up recording, I'm not sure what we'll do for a "Garrett" voice.

Regarding cathedrals with more guards than clergy, I would assume that the clergy go home at night?
