We do that when necessary, sometime when it's obvious the loot is misplaced we move it into a frobbable location, other times we remove it entirely.

having to find a door/compartment, after hitting a switch that is on a timer, where you hear it open but cant find it :P
really hard math problems, where you forced to look it up in online in order to get a code/obscure verbal clue to a code

Theres also some authors that used loot items as decor and somewhere along the way (olddark vs newdark or something?) these loot objects end up being counted on the endstats.
Thankfully the community has dome many DMLs to fix that along the years :D

There was once a clue in one FM: a written phrase that the player should pronounce in English and try to hear these sounds as poorly pronounced digits. And the phrase wasn't something trivial like "for too...". It was the ragest ragequit I have ever done. I didn't finish the mission even after I got the correct code from the walkthrough :)

One thing that does bug me is the use of the same door skins for non-functioning doors, they have the same handles and locks as normal doors but don't do anything. Why, oh why use them?

Guards that don't flinch at grinding metal elevators but get alerted by a lone step rooms away.

Same!
And also having a "Don't kill anything" objective and finding a buttload of offensive weapons like that and none of the things like gas arrows & mines, flashbombs, moss arrows...
grrr!

I've seen instances where I discretely knock out a guard, and 30 seconds later I hear an alarm, or ruckus from the other guards...several rooms away where they couldn't possibly have heard me. I suspect it's just an engine bug
