What about System Shock 2, where smoking cigarettes hurt you, but could never kill you, instead leaving you as a zero health zombie?
TF's SecMod made them restore some psi points, too.

I'm currently playing through Fallout 3 for the first time, and the notion of skipping straight to New Vegas just seems messed up. There is so much to do in FO3, and so many different ways to build a character, that it's hard for me to imagine getting bored with it.
Also, melee + VATS + Bloody Mess = the best thing ever.

I got tired of the excessive amount of gore in that game after a while, and eventually installed a mod to make people gib a little less often. I don't mind a bit of gore, but it was getting excessive to the point of the ridiculous because it happened so often. I shudder to think what it would be like with Bloody Mess.
But then, I didn't use VATS, probably due to being weird and preferring real-time combat.

Something about FO3's realtime combat just doesn't work for me. Maybe it's the slow firing rate of most weapons, or the weird way some enemies slide around when they get close to you.

I've started FO3 a few times trying to build a ninja character, but the inability to target melee hits in VATS really kills it for me.

Eh, by the time you've leveled up enough to get access to the relevant ninja-building perks, most enemies will be going down in one or two punches anyway.

I thought it was because the enemies don't seem to flinch much, and sidestep strangely. They just seem to float around, soaking up damage until arbitrarily dying. I thought it was the weakest part of an otherwise good game.
