I've never had a "heart attack."
In RTC, a haunt was chasing me through the entire level! I was running like mad and my heart was racing, so I jumped off the broken stairway (where you first see friendly ghost) and went into the corner. The haunt stared at me for a minute, then ran away. I pulled my hands away from the keyboard to catch my breath, when suddenly the very same haunt burst through the door directly in front of me.
I screamed and slammed my hands repeatedly onto the keyboard, hitting many keys, but none of them in effect saving my life.

Well, probably the stupidest death to happen to me was actually a series of deaths in the very first mission in Thief I (Bafford Manor, I think it's called). At the beginning when you see the guards at the door I think Garrett says something about not being able to get through those guards. But me, being the total newbie at that time, thought to myself 'Well, I can!' And than I'd proceed into the area, guns (okay, daggers) blazing. I can't tell you how many Garrett souls went to Heaven that day (or days) before I relized 'Hey, I can get AROUND the guards....'

Oddest death was in the Thief 2 demo. I shot an arrow into a wooden area in the roof above the window near the greenhouse to climb up and in. I climbed to the top of the rope and as I was just touching the sky I suddenly was thrown back from the rope at high speed, shot clean over the greenhouse and the area on which it's built, and plummeted into the street below.
Very wierd. Needless to say I was very dead.

My weirdest death? I was sneaking through the Mage Towers in Thief Gold. I got up to one of the interior doors, put out the torch beside it, opened it, and leaned to look left. I examined the guard on that side while I sat in darkness because of the lean. I then turned the opposite way and leaned to look to the right of the door. As I did though, my screen suddenly jerked and I was flung across the giant dining room faster then a Haunt on two speed potions, and smacked into one of the tables with such force then I instantly died.
To this day I've never been able to get that to happen again.

I've thought about another one, too. On of my very favorite Thief moments is in the gigantic room in The Sword. Y'know, the one where the furniture and everything is so huge you feel like a rodent? I took a substantial amount of plummets from the edge of the sink where you enter, trying to get down. Imagine the gossip...
"And - wait till you tell you this part - he died falling off the SINK!..."
Poor Garrett would never live it down (if he was still alive, that is).
Another one would be when a friend and I were playing and we came across a walking zombie for the very first time in "Cregscleft prision" (or whatever it's called). We freaked out and ran in any random direction we came across, running into the exact same zombie 3 times! The 2rd time it must have gotten sick of seeing the same screaming person running past, for it finally killed us.

Mine is the Return to the Haunted Cathedral one, with the stairs. I didn't see it coming, and I hadn't saved until the very first of the level. Oh, the frustration...

Well, I din't got killed, but were near suicide when I really screwed the game up. When first playing Metal Age I thought "heck, lets play it safe and sound and start with 'normal'". When I reached Framed I threw in 'hard' (more interesting goals) and played the whole game through with that. After kicking the dusts of Karras I thought "that was fun. Lets play the first two with 'expert'". After five hours of intensive gaming I hadn't complited the first one! I was lacking the gems needed to complete the mission and got stuck for hours. Garrett got hurt when I piled up crates to search chandeliers and things like that. I had found the lever that opens the secret over the bedroom bed, but hadn't noticed it 'cause it was in the shadows. You can try to guess how I felt when I was reading the walkthrough...
