Why? It's nothing more than a dressed up Zombie level.
Doesnt need to be more than that. The way it is dressed up, I find myself going crouched, sticking to shadows and trying not to breathe or even think too loud in the Outer Cradle - although I have played this several times already and although I know there wont be any zombies around. The entire place feels hostile to me, in every aspect. I dont like being there. People are scared by their own imaginations - not necessarily by what actually is there to be seen and heard. Imagination. The Cradle successfully triggers it. Thats it for me.
Sure this doesnt work for everyone. Apparently it does for many.

I've never had a scary dream about the Cradle. That being said though, I almost never have dreams or at least never remember having them.
I found the first Thief to be scarier as a rule. The zombies, the haunts, missions like the Bonehoard etc.

I ne'er have had a cradle inspired nightmare,
I have had a cradle inspired cool dream
I'm sitting in a half-truck, shooting residents with a MG-42

In this case, I agree with Gvozdika. Its not that the mission itself is 'scary', especially on replays. Its that it triggers the imagination and the whole atmosphere is condusive to sucking you in and letting you imagine all sorts of . . .nasty things.
I just finished playing it for the second time and found myself just as creeped out as the first time, even though I knew what to expect, the fact that there was nothing moving in the first half, and that there were only 9 patients in the second. I knew their stories and what to expect, yet as was said, the place *feels* hostile. It doesn't want you there and once it knows you are, you become trapped.
I found myself slinking around, hoping not to be noticd, nearly screamed when I turned around and found myself face to face with a patient (of course thats not new. . .I've found myself doing that when I run into guards/Hammerites unexpectedly too, but in this environment it was a bit more of a shriek lol) For me, I think its a combination of the sounds around me, the feel of the place, the story and the fact that my imagination starts filling things in to make it worse. (I mean, a woman carrying around the ashes of her dead baby? ew! Or. . .*why* was a little girl's nightgown in this patient's room? Why did he take it? Thats just. . .creepy) I can't say its SCARY, so much as creepy. This was not an orphanage. . .this was not a hospital. . .this was a place for so-called doctors to cut up live people and experiment, while children were also experimented on (drawing of blood, etc) and allowed to/made to deal with the mentally sick. The whole premise is very disturbing.
That being said, no, I have not had any nightmares inspired by this place, despite the fact I only seem to get to play it very late at night before I go to bed. Nightmare/dream about Return to the Cathedral, yes - but not the Cradle. (Oddly, the Cathedral did not creep me out much at all.)

It was probably the most enjoyable undead mission I've played (and I hate other zombie levels with a passion - believe me...). The story really isn't what scares me, as I'm a huge fan of horror movies, and this story isn't that fazing. But the monsters themselves freak me out...fast little bastards who fly at you shreaking their heads off... I hate how if you kill them in the dark, than accidently step on them, they come back to life and scare the sh*t out of you. I hade to take a few break from this game before I first finished it;-) But once you get through it at least 1 time, it's not as bad.
