I honestly can't think of any game music I outright despise, though I agree with Koki about the Chrono Cross battle track. I still believe the OST's one of the best ever composed apart from that track, however, and that's unlikely to change very soon.
So anyway, if it's any consolation, I routinely wake sleepy mates up to Cave Story's unabashedly ear-grating (

brilliant alarm clock theme. :ebil:

I'd never really thought about it before, but it is kind of ironic that the worst track in Cave Story is the title theme.
(unlike Deus Ex, which has an amazing kickass title theme)

Absolutely agree on the Chrono Cross battle theme, and yes, I do love the rest of the soundtrack as well. Oddly enough, when I first played Quake 2, I somehow liked the music. Now I agree with everyone else here.
I think I played a couple indie games recently that had pretty terrible music, but I can't remember which ones those were.
The Incredible Machine 3 has a very bipolar soundtrack. I find I hate about half the tracks, but also really like a few as well.
Kuru Kuru Rock from the We Love Katamari soundtrack was abysmal.
Much as I like the game, the music from Kohan is absolutely generic and usually gets turned off. Same goes for Geometry wars.
I'm sure there are more, but they don't come to mind and I can't exactly search for albums that I've completely deleted.
